You answer it and discover the PCH Prize Patrol at your doorstep with a giant bouquet of red roses, a sea of multi-colored balloons, a bottle of champagne and a “BIG CHECK” with your name on it! Once the excitement quiets down, but before you can take the real check to the bank and go on that long-awaited vacation or start paying off your bills, there’s just one thing you need to do — sign a Sweepstakes Winners Affidavit Of Eligibility. What is that, you might ask? It’s an important document that helps ensure that a prize is being awarded to the true and rightful winner, the owner of the winning SuperPrize Number.

You see, an affidavit is a legal document that verifies a person’s personal information, such as name, age and social security number. Affidavits are witnessed by a notary public, who is responsible for ensuring this information’s authenticity. This is why winners of PCH prizes over $600 are required to complete a Sweepstakes Winners Affidavit of Eligibility as indicated in our sweepstakes Official Rules.

The Sweepstakes Winners Affidavit of Eligibility also ensures that a winner meets PCH eligibility requirements, such as:

  • The winner is not a principal or employee of PCH, its affiliates, subsidiaries or contest processors or one of their immediate relatives, or a member of the board of judges.
  • The winner consents to having his or her name and likeness used to help us make the general public aware PCH awards prizes as offered.

Once a prize is awarded, PCH Winners have 30 days to complete the Sweepstakes Winners Affidavit of Eligibility, have it notarized and forward it to PCH Headquarters in Port Washington, New York. Of course, our winners are all too happy to sign and return it! After all, big prizes are at stake!
