I got a final step package in the mail. Do I have to send form c2579a in with #1 form, or do I hold on to it for reference? And do I need a w-3 form, or is form #1 all I send to claim the #19500 prize of $1,000.000 and 5000 a week for life, for me and someone I pick to get $5000 a week for life?

I’ve received this package in October and sent to PCH everything that I’ve needed to before November 4th, you must have everything.
I claim final step complete compliance 19500. $10K a week for life upgrade approved. I’ve searched, I’ve claimed, I activated, and take sole ownership of prize number **** ****9815 and **** **** 2714 and all other timely entries I’ve submitted.
