Picture this, PCH Fans: You are home and you hear a knock at the door. You answer it and discover the PCH Prize Patrol at your doorstep with a giant bouquet of red roses, a sea of multi-colored balloons, a bottle of champagne and a “BIG CHECK” with your name on it! Once the excitement quiets down, but before you can take the real check to the bank and go on that long-awaited vacation or start paying off your bills, there’s just one thing you need to do — sign a Sweepstakes Winners Affidavit Of Eligibility. What is that, you might ask? It’s an important document that helps ensure that a prize is being awarded to the true and rightful winner, the owner of the winning SuperPrize Number.
You see, an affidavit is a legal document that verifies a person’s personal information, such as name, age and social security number. Affidavits are witnessed by a notary public, who is responsible for ensuring this information’s authenticity. This is why winners of PCH prizes over $600 are required to complete a Sweepstakes Winners Affidavit of Eligibility as indicated in our sweepstakes Official Rules.
The Sweepstakes Winners Affidavit of Eligibility also ensures that a winner meets PCH eligibility requirements, such as:
- The winner is not a principal or employee of PCH, its affiliates, subsidiaries or contest processors or one of their immediate relatives, or a member of the board of judges.
- The winner consents to having his or her name and likeness used to help us make the general public aware PCH awards prizes as offered.
Once a prize is awarded, PCH Winners have 30 days to complete the Sweepstakes Winners Affidavit of Eligibility, have it notarized and forward it to PCH Headquarters in Port Washington, New York. Of course, our winners are all too happy to sign and return it! After all, big prizes are at stake!
Fans, the fact is — awarding big PCH prizes may be fun, but it’s also serious business. There’s no purchase or fee necessary to claim a prize, but the Sweepstakes Winners Affidavit of Eligibility is one step in the process to ensure that the correct winner is identified, all protocols are followed and everything is done “by the book!” At PCH we wouldn’t have it any other way!
Keep entering for your chance to win BIG!
All the best,
Elaina R.

How Is The PCH SuperPrize Winner Selected?
Hi, I received an affidavit form today through the mail. How do I know it’s the real form PCH sends out? It’s telling me to put in my social security number and have it notarized to claim my prize. I’m hoping this is not a scam.

I am claiming my superprize numbers — thank you pch!!
Excuse me Publishers Clearing House This is highly not possible because my prize numbers is #8212. I Terry am rightful owner of theses numbers! Please investigate!!!
Hello Terry, What you have there is a Giveaway Number. A giveaway number identifies the prize promotional pool into which sweepstakes entries will be submitted. Throughout the life of a giveaway, you may submit many sweepstakes entries, and each one of them will have a new, unique prize number. But, the giveaway will always have the same number. Even though it’s all done by the computer, imagine that each giveaway is a huge barrel, that can contain more than one sweepstakes. And every sweepstakes entry you submit gets thrown in the barrel for its chance to win in the drawing. The barrel will always have the same number, but each entry you throw in the barrel is numbered with a new and different prize number.
Yes, I was informed that I had won and that they would require my Social Security number.
